When you submit a public inquiry form on this website, the information you enter (such as your name, work email, organization and message) is submitted to the third-party FormSubmit service, which is configured to forward inquiries to our testing inbox at saleemyagzan@gmail.com. The form does not open a draft email. Delivery may require initial confirmation of the receiving address.
We use submitted information to respond to business, healthcare practice, institutional partnership, or careers inquiries. Do not provide patient information, medical records, insurance identifiers, confidential institutional documents or other sensitive data through this form. The form and test inbox are not approved channels for protected health information.
Website hosting and the form delivery provider may process technical metadata and traffic logs according to their policies. We have not implemented a patient portal or connected clinical records. Requests to review, correct or delete ordinary inquiry data can be made to saleemyagzan@gmail.com, subject to applicable law and operational limits.
Our legal entity details, data retention practice, overseas data transfers and contact details require final review before full public launch. This page is not a HIPAA Business Associate Agreement or substitute for service-specific security obligations.
